If you’re planning time away from the office for health-related issues (FMLA, Short-Term Disability, etc), or civic commitments (military leave, jury duty, etc) below are the steps you should follow for requesting time off, instead of entering it onto your timesheet:
1. Complete the online form, making sure to include dates and any other required information.
2. Once the form is filled out completely and the supporting document(s) have been attached, click the submit button.
3. After you submit the form, at the top of the page you will see: “Your request has been successfully submitted.”
4. The form is then received by an HR team member who will review it and contact you within 2 business days to approve the request or follow-up with questions.
